What is Blood Flow Restricted Rehabilitation (BFR)?

Blood Flow Restricted Rehabilitation (BFR) is a technique that involves briefly and intermittently restricting venous blood flow using a specialized tourniquet while exercising. This allows patients to train with significantly lighter weights while still achieving muscle hypertrophy (growth) and strength gains. Traditionally, similar results would require lifting much heavier loads.

Application

The goal of BFR is to reduce blood flow in the targeted limb just enough to promote beneficial physiological changes. The required pressure varies based on individual factors, including limb size, tissue density, blood pressure, and tourniquet cuff width. To ensure safety and effectiveness, we use a specialized tourniquet system with built-in monitoring to account for these variables. Before beginning treatment, we will assess potential contraindications to determine whether BFR is appropriate for you.

Training

Once we establish your personalized pressure setting, we will guide you through specific exercises tailored to your rehabilitation needs. Although you will be lifting light weights, your muscles will quickly feel as though they are working hard—this is expected and necessary for achieving optimal results. It is important to complete the prescribed sets and repetitions to maximize the benefits. You may experience sweating and an increased heart rate, which are normal responses to the treatment.

Effects

Following a BFR session, the buildup of lactate in the muscles can lead to several positive physiological effects, including an increase in growth hormone and other anabolic muscle growth factors. This process enhances muscle protein synthesis, ultimately promoting muscle growth and strength.

Side Effects

Adverse effects from BFR are rare. The most common side effects include temporary limb swelling, muscle fatigue, and soreness, which typically resolve within 24 hours. If you experience prolonged swelling, fatigue, or soreness, or have any other concerns, please discuss them with our physical therapy staff.
